Step 1 is mandatory before Step 2. Without behaviour change and supragingival biofilm control, subgingival instrumentation outcomes are compromised.

Behaviour change interventions targeting risk factors should be implemented as part of the first step of therapy.

Smoking cessation interventions should be offered to all patients who smoke.

Diabetes-mellitus control interventions should be offered to all patients with diabetes mellitus.

Professional mechanical plaque removal (PMPR) is recommended for all patients to remove supragingival plaque and calculus and any plaque-retentive factors.
1 Behaviour change interventions
Address modifiable risk factors: motivational interviewing for OH, smoking cessation (R4.2), diabetes control reinforcement (R4.3). Document risk profile (R4.4).

2 Self-performed mechanical plaque control
Tailored OH instructions including interdental brushes / floss as appropriate (R4.5). Adjunctive chemical agents may support short-term plaque control (R4.6).

3 Supragingival professional mechanical plaque removal (PMPR)
Full-mouth supragingival biofilm + calculus removal (R4.7). Required foundation before any subgingival instrumentation.
